I "Browse" to locate the file, I open it.

Browse-locate file-open file-then go to bottom of page and post message

That's exactly what I have done. Three times the message has posted without a file attached.
Once I tried on a forum thread that was sort of idle, so that I wouldn't be on a bust thread, and I "inserted" a pic as well as attaching a pic. (Actually I did not insert a pic; it just made a red X.) That one time the file did attach.

Make sure the file extension is in lowercase letters.

I think uppercase works now Waste. I had a major problem attaching files when I first joined the site, and after several PM's with Diz-X he determined that it was a lowercase-uppercase issue. He then fixed the problem.
