I'm more curious about if just about any tattoo artist will do those tats or if you have to find a specialist. Do people just walk in off the street and go "Hi! I want to have a butterfly tattooed around my anus."?

I think you would have to find someone who specializes in that sort of work. Any good tattoo artists will know what he/she feels comfortable doing and would rather tell someone "Can't do it" than attempt something ridiculous.
